Your initial evaluation isn’t rushed — it’s a full 60-minute conversation designed to understand the whole picture. We’ll talk through what you’ve been feeling, your personal and family history, what’s worked in the past (and what hasn’t), and what you’re hoping to change. By the end, you’ll have a clear, personalized plan — and someone in your corner moving forward.
These ongoing sessions are designed to keep things aligned — your medication, your mood, and your goals.We’ll check in on how you’re feeling, make any needed adjustments to your treatment plan, and talk through what’s working (and what’s not). If you’re in therapy, we’ll support the work you’re doing there, too — so everything stays connected. It’s low-pressure, judgment-free support to help you stay balanced and moving forward.

These tools are not tests you can “pass” or “fail” — they are simply a way to guide our conversations and track your progress over time. They help us see the big picture so we can create the best treatment plan for you.

 

For Depression and Mood

  • PHQ-9 (Patient Health Questionnaire-9)
    A short set of questions about mood, energy, sleep, appetite, and focus over the past two weeks. It helps us understand if you might be experiencing depression and how it’s affecting daily life.
  • MDQ (Mood Disorder Questionnaire)
    Looks for patterns of mood changes — times when you’ve felt unusually “up” or energized, and times you’ve felt low — to see if mood swings may be part of a bipolar pattern.

 

For Anxiety

  • GAD-7 (Generalized Anxiety Disorder-7)
    A brief checklist about worry, restlessness, and difficulty relaxing. It helps us understand the level of anxiety you may be experiencing.
  • PCL-5 (PTSD Checklist)
    Looks at symptoms that can happen after a traumatic event, like unwanted memories, feeling “on edge,” or avoiding certain situations. This helps us know how trauma may be affecting you.
  • SCARED (Screen for Child Anxiety Related Disorders)
    Designed for children and teens, it helps identify different types of anxiety, such as separation anxiety, social anxiety, or panic.

 

For Attention and Focus

  • Conners Rating Scales
    Helps assess attention, focus, and activity level. Versions can be filled out by you, a parent, or a teacher to get a complete picture.
  • ASRS (Adult ADHD Self-Report Scale)
    A simple self-check to see if ADHD symptoms may be affecting focus, organization, or follow-through in adults.
  • BRIEF (Behavior Rating Inventory of Executive Function)
    Looks at everyday thinking skills like planning, staying organized, and managing emotions.

    Helps us understand if you’ve had thoughts about harming yourself so we can make sure you have the right support and safety plan.

Telehealth makes mental health care more accessible, comfortable, and convenient. You can meet with your provider from your home, office, or anywhere you have privacy — no commute, no waiting room. Still, preparing for your telehealth appointment is important to get the most out of your session.

Here’s how you can set yourself up for a smooth, productive visit:

1. Find a Private, Comfortable Space

Choose a location where you feel safe and won’t be interrupted. Close the door, reduce background noise, and let others know you’ll need privacy. A quiet space helps you focus on the conversation and ensures confidentiality.

2. Test Your Technology Ahead of Time

About 10–15 minutes before your appointment:

  • Make sure your device is fully charged or plugged in
  • Check your internet connection for stability
  • Test your camera and microphone
  • Have your telehealth link ready to click at your scheduled time.

3. Have Your Information Ready

Write down your:

  • Current medications (including supplements)
  • Allergies / food intolerances
  • Any recent health changes
  • A brief summary of your symptoms, concerns, or recent life events
  • Pharmacy information for prescriptions

Having this handy helps your provider make informed decisions during your visit.

4. Prepare Your Questions and Goals

Think about what you want to accomplish in the appointment. Are you looking for help managing a new symptom? Considering medication adjustments? Wanting advice on lifestyle changes? Writing these down ensures nothing gets overlooked during your session.

5. Minimize Distractions

  • Silence your phone notifications
